ESSENTIAL POSITION FUNCTIONS
 
A.Job knowledge and Role Responsibilities:
 
1.Follows established philosophy and objectives, policy and procedures of nursing service and contributes to change when necessary. 
2.Demonstrates knowledge of age specific development factors specific to adult and geriatric residents (i.e, physical, cognitive and socialization factors) in planning delivery of care. 
3.Demonstrates knowledge, skills and techniques necessary to care for residents with the following needs: alzheimers, dementia, dialysis, IV therapy, infectious diseases, wound care, and all disease processes if applicable. 
4.Provides nursing care in accordance with Resident Care Policies and Procedures and ensures the safety and well-being of the residents is maintained. 
5.Maintains and updates license in compliance with State regulations. 
6.Acts appropriately under the direction of the Director of Nursing Service or Supervisor and acts as an active member of the interdisciplinary team. 
7.Demonstrates ability to adjust to change in unit/shift assignments to meet resident and facility needs. 
8.Responds appropriately to behavioral, medical, and fire safety situations. 
9.Demonstrates ability to adjust to changes in unit/shift assignments to meet resident and facility needs. 
10.Responds appropriately to behavioral, medical, and fire safety situations. 
11.Aware of Resident Abuse Reporting and Prevention Policies. 
12.Demonstrates knowledge of and complies with the Code of Conduct and Compliance Program when performing work functions. 
 
B.Resident Care Responsibilities:
 
1.Plans effective administration of nursing care in cooperation with other health related disciplines on each unit. 
2.Responsible for assessment of resident needs, development and implementation of comprehensive care plan. Consults with residents’ physician in planning resident care. Periodically reviews and updates comprehensive care plan. 
3.Evaluates incidents and determines emergency measures to be used. Assesses and directs emergency treatment to residents and staff involved in accidents. If necessary, notifies the attending physicians. Completes the incident report with causes investigated and corrective actions taken and/or suitable recommendations made to void recurrence. Updates the care plan with new interventions. 
4.Supervise planning of daily assignments for personnel on the unit. 
5.Confirms with established policies and procedures and maintains staff awareness of these in their daily work. 
6.Schedules and leads staff conferences to promote initiative and solve resident care oriented problems. 
7.Works with interdisciplinary team members in developing a discharge plan for individual residents and assists in achieving discharge planning. 
8.Assists in orienting and directing new staff. 
9.Maintains accurate, adequate records of resident care and related statistics as required. 
10.Reviews and interprets work performance against accepted standards. Responsible for completion of performance evaluations of a daytime nursing staff. May review and provide input for performance evaluations for nursing staff on alternate shifts. Completes proficiency audits for nurses and assigns and reviews audits for Certified Nursing Assistants. 
11.Directs requisitions of equipment and supplies and encourages proper utilization of same. 
12.May provide input to the In-service Coordinator in planning nursing in-service programs and on-the-unit conferences based upon an assessment of staff’s learning needs and/or resident care problems. 
13.Handles resident or staff grievances and/or refers them to the appropriate department supervisor. 
14.Assist in establishing and maintaining a positive staff and good public relations. 
15.Is available for emergencies and assistance to nursing staff as needed. 
16.Communicates and acts as a liaison among residents, family, physicians, and staff. 
17.Accompanies physicians on rounds, examinations and treatments, as needed. 
18.Promotes and maintains safe work performance by staff. 
19.Informs the nursing supervisor and director of nursing services of pertinent information concerning resident status, staff actions, employee attitudes and suggestions. 
20.Conducts resident rounds to assess and/pr supervise nursing care, assess condition of residents, and safe, clean, comfortable environment. 
21.Receives reports from personnel on previous tour of duty and ensures that the continuity of care continues by staff on next tour. 
22.Participates in Nursing Management Meetings, HART Team Meetings, and Comprehensive Care Plan Conferences. 
23.May provide input in formulating nursing policies and procedures. 
24.Notifies the resident’s physician, family and/or health care agent of any change in the resident’s condition. 
25.Continues professional growth through reading current professional literature and attending professional meetings, seminars, and workshops. 
26.May perform duties listed on job description for staff nurse in his/her absence. 
27.Audits staff nurses to ensure that all resident care systems and processes are followed per policy. 
28.Perform other duties as required. 
29.Demonstrates ability to perform all physical and mental job functions of a Certified Nursing Assistant, Licensed Practical Nurse and Registered Nurse. 
 
C.Role Responsibilities – Resident Care Systems:
 
1.Systems are to be established and consistent throughout facility. These systems are to be spot checked on each tour of duty. These include but are not limited to ( as per standardized procedure): 
a.Unit management systems for group assignments and Nursing Assistant responsibilities on a Daily Unit Management form. 
b.Scheduling of nurses for employee meals or breaks, ensuring that breaks are not taken while residents are dining and is to be recorded on the Daily Unit Management form. 
c.Restraints to be kept to a minimum and are released at least every two (2) hours, or according to established Care Plan. Restraint releases are to occur on the unit at the same time or per Care Plan, and observed by a licensed staff member. 
d.All residents are to be out of bed by noon, or per Care Plan, unless a specific physician order indicates bed rest. 
e.Turning and positioning schedules are followed every two hours ( or per Care Plan). The turning and positioning system is to be followed so that it can easily monitored. 
f.Resident ambulation occurs as an observed system (ie., before the meal period in the dining room, walk-to-dine, etc.). 
g.Conduct periodic reviews so that supplements are passed according to developed schedule. 
h.Checks residents during meal time to ensure that staff are offering substitutes / supplements, and proper documentation is occurring. 
i.Checks hydration cart is passed per schedule. 
j.Infection control and exposure control plans are implemented according to Resident Care Policies and procedures. 
k.Conducts periodic reviews so that environment is clean, orderly, and organized. Cleaning schedule are in writing and audited daily. 
l.Change of Condition / 24-Hour Report is completed daily. Weekly head-to-toe audits of skin documentation are done. 
m.Demonstrates ability to perform all physical and mental job functions of a Certified Nursing Assistant, Licensed Practical Nurse, and Registered Nurse. 
 
D.Role Responsibilities – Resident Care Documentation:
 
1. Nursing documentation is to meet the following standards: 
a.Care plans match Daily Guides, Treatment Sheets, and Physician Orders. 
b.Maintains and evaluates statistics for: pressure sores, resident Accidents and Incidents, number N/G or G tubes, number of justified foley catheters, resident census, resident and employee infections. 
c.Reviews all closed charts of expired resident and ensures appropriate documentation by nursing during the last 48 hours and discharge orders by physicians are enclosed (to contain how and where discharge – i.e. to funeral home via stretcher). Reviews discharge summaries of all residents. 
d.Conducts periodic reviews of Care Plans to ensure they are interdisciplinary and updated by the Charge Nurse as changes occur. 
e.Ensures all communication to nursing by other disciplines are flagged and the Charge Nurse is responsible for implementing the changes on all forms. 
f.Periodically check for documentation for residents on 24-Hour Report. 
g.Periodically checks for skilled documentation for Medicare services. 
h.Checks on each unit for completed MARG, TARS, and flow forms each tour of daily 
 
E.Role Responsibilities - Interpersonal Skills:
 
1.Is aware of, and adheres to, resident’s Bill of Rights and Confidentiality of Resident Information. 
2.Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner. 
3.Addresses family satisfaction issues immediately and assists in resolving the matter in a professional matter. 
4.Is aware of Resident Abuse Reporting Law. 
5.Participates as assigned in the orientation of new staff and demonstrates a positive, helpful and enthusiastic attitude. 
 
F.Role Responsibilities – Safety:
 
1.Understands and can demonstrate knowledge of role in Fire and Disaster drills. 
2.Demonstrates ability to locate emergency equipment (i.e. emergency oxygen tank). 
3.Demonstrates ability to identify / respond appropriately to potential behavioral outbursts. 
4.Uses facility equipment safely. Understands how to lifts appropriately and practices good transfer techniques. 
5.Recognizes, removes, and/or reports potential hazards. 
6.Understands OSHA recommendations. 
7.Is knowledge of MSFS labels hazardous chemicals. 
 
G.Resident Care Responsibilities – Infection Control :
 
1.Washes hands between resident contacts as appropriate. 
2.Demonstrates knowledge of and implements Standard Precautions. 
3.Handles linens and disposes of water properly. 
4.Demonstrates ability to care for residents in precautions. 
5.Demonstrates knowledge of correct positioning / care of catheters and drainage. 
6.Understands how to handle infectious waste. 
 
H.Resident Care Responsibilities – Resident Dignity:
 
1.Addresses residents in a respectful manner. 
2.Discusses confidential resident information in appropriate areas only. 
3.Understands HIPAA regulations and maintaining confidentiality of medical information. 
4.Knocks on door prior to entering resident rooms, and maintains resident privacy and dignity when providing treatments or other resident care.

SUMMARY OF QUALIFICATIONS:
1.Maintains a current, valid License as a registered professional nurse in good standing. (Maintains a current valid Nursing License as a licensed practical nurse in good standing). 
2.Possesses a minimum one (1) year of experience in nursing service administration. Additional education or experience in such areas as rehabilitative or geriatric nursing is preferred. 
3.Possesses strong knowledge of state, federal and local regulations as they pertain to long term care. 
4.Is employed full-time and serves only one facility in such capacity. 
5.Maintains rapport with outside medical professionals. 
6.Successfully completes facility conducted orientation, mandatory training and in-service programs. 
7.Must meet health assessment requirements, including two stage Mantoux skin test. 
8.Must be capable of performing the essential functions of the job, with or without reasonable accommodations. 
9.Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.
