As an APS Health Coach/Case Manager you will provide support and education to people with health and wellness concerns, who suffer from acute and/or chronic disease or at risk for the development of a chronic disease.
Our Health Coaches perform assessments and interventions to connect with members and positively affect their care and quality of life. Activities and interventions are conducted in a systematic approach and include guiding, facilitating and coordinating the care for the high-risk targeted members in an appropriate, efficient and cost-effective manner.
Job Accountabilities:
1.Reviews significant information, when available, from the medical history and physical, laboratory data, pharmacy information, and other diagnostic testing results, screens and assessments.
2.Identifies problems, sets goals, interventions and determines the member’s health status based on assessment.
3.Establishes goals/care plan for each member regarding educational and health care needs.
4.Provide education and support that empowers members to independently navigate the health care system, understand their disease process and manage it effectively.
5.Communicates with primary care and/or treating physician to request specific health information and/or to provide an update of a member’s health care status.
6.Collaborates with member’s health professional team, e.g. certified dieticians, behavioral health specialists, respiratory and/or physical therapists.
7.Identifies and facilitates transfers of appropriate cases needing more intensive management to case management.
8.Performs follow up assessments and makes contact with a member in a disease management program as appropriate.
9.Prepares reports and case summaries as needed.
10.Completes special assignments and projects instrumental to the function of the department.
11.Works in conjunction with operational functions regarding administrative staff.
12.Limited local travel.
13.Other duties as assigned.
EDUCATION:
Medical knowledge and background obtained by completion and licensure as a registered nurse or equivalent behavioral or medical health care professional is required.
REQUIREMENTS:
A minimum of three years active clinical experience in an acute care environment is required. Experiences teaching in a clinical setting to patients are desired, as is experience in collaborating with all members of the health care team. Previous knowledge of health insurance, or managed care is desired. Experience in home health care preferred. Excellent oral and written communication, written report preparation and interpretation, human relations, and decision-making skills are required.
