Position is a member of a multi-disciplinary team and will assess, plan and coordinate the behavioral health care needs for adult clients with severe and persistent mental illness including rehabilitative treatment and support services.
You will perform assertive outreach and case management services and will coordinate member referrals to appropriate community services and participate in on-call 24 hour rotations for emergency response.
Bachelor’s Degree with minimum 12 credit hours in a social science field plus 2 years related experience.
Minimum of two years related experience with adult SMI population.
Knowledge of hospital procedures governing the discharge of mentally ill.
Ability to maintain complete and accurate member records and to establish rapport and work effectively with adult SMI members.
Independent, highly organized team player, able to handle multiple tasks simultaneously.
Excellent communication skills.
Computer proficiency a must.
Clean driver’s abstract and access to vehicle a must.
