Bachelor of Science in Nursing. Current, active, and unrestricted license as a Registered Nurse in the State of Ohio is required. BCLS under the American Heart Association guidelines is required. Valid Ohio Driver's License
One year of Dialysis or Transplant Nursing experience.
The position scope of the Nephrology Case Manager includes but is not limited to the following abilities:
Clinically oversee care of dialysis patients
Clinically coordinate care of transplant patients
Coordinate clinical management of renal and transplant patients
With physician partners
Identify quality issues related to care of renal and transplant population.
