Responsible for evaluating patients referred for home health services. Manages and facilitates the transition of patients into the home care program in accordance with applicable laws, regulations and agency policies and procedures. Markets home care services to multiple disciplines, including physicians, social workers and case managers. Establishes a plan of care in collaboration with patient, physician and members of interdisciplinary healthcare team.

Job Requirements:

 

 

Associates/Diploma from an accredited school of Nursing, Bachelors Degree preferred. Minimum of one year of acute, medical-surgical nursing experience. 2 years CHHA/LTHHCP experience required. Knowledge of Word and Excel preferred. Licensed with current Registration to practice as Registered Professional Nurse in NYS. Excellent customer service skills. Ability to work within an integrated health care team, demonstrate effective critical thinking skills and problem solve independently. Ability to communicate in English both verbally and in writing. Proficiency in interviewing and physical assessment skills.
