Job Position Details

Job Title: Administrative Assistant/Scheduler 32 or 53 hours PP

The Scheduler is responsible for shift intake process, scheduling client services, maintaining accurate client and co-worker files, timely and accurate entry of a variety of data including client information, HR information, bill and payroll information, establishing positive relationships with customers and referral sources and routine clerical matters. Other relevant duties as may be required or assigned. 

 

 

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 

 

Responsibilities of the 3-11 Scheduler include, but are not limited to, the following:Accurately enters data into HomeTrak on a daily basis and verifies accuracy of data entered, including;Client referral information (including client group, notes and source information) HCA mileage and travel time New client information to include due dates for the following:Quality Assurance due dates (one and three-week phone surveys, 3-month written survey) Updated care plans Reassessments New co-worker demographic information and due dates for the following:Annual Mantoux and physical Annual competency training Supervisory visits Driver's license renewal Auto insurance renewal CPR Verifies and/or corrects on a daily basis:Telephony information for call-ins and call-outs to ensure that they match schedule. Calls HCAs and/or clients to clear up discrepancies. Writes schedule notes to explain discrepancies such as reasons for arriving late or leaving early, etc. Open calls 

 

Handles all telephone calls regarding scheduling and coordination of home care associates to meet client scheduling needs. 

 Answers incoming calls in a warm, pleasant and professional manner Responds to message content appropriately Fills out intake forms as inquiries come in and passes information on to GCS on a daily basi Monitors HCA schedules to ensure timely arrival at assignments through telephony system. Schedules home care associates. Notifies supervisor regarding problems in scheduling or actions requiring disciplinary action. Completes quality assurance surveys as directedCompletes phone surveys at one week and three weeks after start of care Mails out written surveys after three months from start of care and upon discharg Prepares HCA picture ID and insures that HCAs receive ID before accepting client assignments Orders HCA name tags Accurately prepares and maintains files for co-workers and clients:Files elements of records on a daily basis Notifies appropriate supervisor of missing information Closes client records after discharge Notifies ED of discharge in order to process deposit refund 

? Maintains adequate supply of client and co-worker record elements Prepares new co-worker orientation packets Prepares HCA folders Prepares new client welcome packets Copies memory support material as directe Communicates with Client Service Coordinator and/or 7-3 scheduler on a daily basis regarding all new inquiries and scheduling activity or changes. Maintains and protects client and coworker confidentiality. 

 

Other duties as required and assigned.
